Welcome! To help our initial visit be as efficient as possible, please download and complete the new patient form. It can be a bit lengthy, but taking the time to fill it out ensures that we spend your appointment focused on the most important issues.

Feel free to mail it to me ahead of time or bring it to your appointment. Please let me know if you do not have a working printer, and I will have the form ready for you 15 minutes before your appointment.

Please provide 24 hours notice if you need to change or cancel an appointment; my missed appointment charge is $30. Along with accepting new patients, I also accept Visa and MasterCard.
